CASE MANAGER – Residential Family Program

1. QUALIFICATIONS
The individual holding the position of Case Manager must be a mature Christian
adult with personal integrity, character and reputation consistent with the standards and
ethics expected of BCV staff. This individual must possess, as a minimum, two years of
case management experience and a Bachelor’s degree in a social or behavioral science
field. The person in this position must have the ability to effectively communicate, exhibit
sound judgment, assertiveness, and be able to work effectively with residents, families,
referral sources and BCV staff. This individual must have a relatively flexible schedule
and be available to work when residents are on campus or when their families may be
available.

2. DUTIES
a. Assist in conducting new admissions and discharges.
b. Serve as a member of the Program Team.
c. Develop the IPOC with residents upon admission, review quarterly.
d. Assume responsibility for documentation of case management and other
relevant clinical information required for the case file.
e. Assist residents in applying for and obtaining resources such as SNAP, WIC,
childcare, mental health services..etc.
f. Assist residents in scheduling appointments and transportation to appointments
as needed.
g. Assign educational materials to residents utilizing BrightCourse online
education program. Meet weekly with residents to review progress and discuss lessons.
h. Represent The Baptist Children’s Village at any review held by public authority
involving residents in caseload.
i. Write weekly reports on resident progress and needs.
j. Assist with TBRI Implementation.
k. Provide aftercare services for families as needed.
l. Other duties as requested by the supervisor.

3. SUPERVISION
This position reports to the Program Manager.
